Job Summary:

As a Community Resource Coordinator II at Centene Corporation, you will play a vital role in transforming the health of our communities. Your primary responsibility will be to connect members with community resources, supporting their care management journey and providing necessary care resources in a cost-effective manner. You will serve as a liaison between members and healthcare providers, promoting awareness of care services and facilitating successful community connections.

Key Responsibilities:

• Provide support to members to connect them to known community and care resources in a cost-effective manner
• Serve as a support for members on community and care resource inquiries and opportunities available to members
• Document and maintain all community resources to ensure standards of practice and policies in accordance with health plan requirements
• Provide assistance to the clinical team of nurses and social workers
• Conduct non-clinical general health assessments to refer members to appropriate care/services
• Conduct non-medical assessments to refer members to appropriate care/services
  Requirements:
  
• Requires a High School diploma or GED
• Requires 1-2 years of related experience
• Up to 50% local field-based travel
